ABSTRACT

Resumo Relatar um caso de um paciente portador de Oftalmia Simpática (OS), com descolamento seroso da retina documentado através de tomografia de coerência óptica de domínio spectral (SD OCT), indocianina verde (ICG) e angiofluoreceinografia (AGF), que o diagnóstico foi realizado em uma consulta de rotina e iniciado tratamento clínico .


Abstract To report the case of a patient with sympathetic ophthalmia (OS), with serous detachment of retinal documented by spectral domain optical coherence tomography (OCT), indocyanine green (ICG) and angiofluorecephography (AGF). The diagnosis was made in a routine consultation and clinical treatment was initiated.

ABSTRACT

ABSTRACT Microsurgical subinguinal varicocelectomy (MSV) is generally considered the gold standard nowadays in view of the lower risk of complications and recurrence. To achieve complete ligation of veins while preserving testicular artery (TA) during the procedure remains challenging despite the application of high power optical magnification and micro-Doppler ultrasonography. The use of intraoperative indocyanine green angiography (ICGA) with infrared fluorescence operative micro-scope in MSV potentially lowers the incidence of TA injury and shortens the learning curve of novice surgeons. We present our initial experience in the application of the technique in nine patients and explore the potential of the new adjunct.

ABSTRACT

A patient with a complete right ureteral triplication presented with recurrent pyelonephritis and flank pain that was refractory to medical management. Evaluation showed that the atrophic upper-most renal moiety had been chronically obstructed and was associated with a dilated ureter. Intraureteral and intravenous indocyanine green (ICG) were used as real-time contrast agents intraoperatively to facilitate right robotic partial nephroureterectomy of the diseased system. Intraureteral ICG was used to accurately distinguish the pathologic ureter and associated renal pelvis from its normal counterparts. Intravenous ICG was used to assess perfusion in the right kidney and delineate the margins of diseased renal parenchyma.

ABSTRACT

PURPOSE: To compare clinical, angiographic, and optical coherence tomographic characteristics between eyes with acute Vogt-Koyanagi-Harada (VKH) disease and eyes with acute bilateral central serous chorioretinopathy (CSC), and to demonstrate distinguishing features between the two diseases in confusing cases. METHODS: The medical records of 35 patients with VKH disease and 25 patients with bilateral CSC were retrospectively reviewed. Characteristics according to slit-lamp biomicroscopy, ophthalmoscopy, fundus photography, fluorescein angiography, indocyanine green angiography, and spectral-domain optical coherence tomography were compared between the two diseases. RESULTS: Five of 35 patients (10 of 70 eyes, 14.3%) with VKH disease were initially misdiagnosed as CSC patients, and six of 25 patients (12 of 50 eyes, 24%) with bilateral CSC were initially misdiagnosed as patients with VKH disease. Pigment epithelial detachment in CSC and optic disc hyperemia in VKH disease show the highest positive predictive values of 100% for each disease. CONCLUSIONS: Optic disc hyperemia in VKH disease and pigment epithelial detachment in bilateral CSC are the most specific clinical manifestations of each disease at initial patient presentation.

ABSTRACT

Vitrectomy is a surgery that involves complex and delicate techniques that treat diseases such as macular hole, epiretinal membrane and diabetic macular edema. Chromovitrectomy is one of these techniques and includes the use of coloring agents such as vital dyes or crystals to enhanced visibility of transparent structures during vitrectomy. The aim of this study was to present a modern approach, based on scientific evidence, about the application and indication of vital coloring agents during vitrectomy. The use of such agents has made this surgery more predictable and has increased its post-operative prognosis. Although research on chromovitrectomy is currently expanding there is still not an established gold standard dyeing agent.


A cirurgia vitreorretiniana é uma cirurgia que envolve técnicas complexas e delicadas que tratam doenças como buraco macular, membrana epirretiniana e o edema macular diabético. A cromovitrectomia é uma dessas técnicas que incluem o uso de corantes compostos de pigmentos vitais ou cristais para melhorar a visibilização de estruturas transparentes durante a cirurgia de vitrectomia. O objetivo desse artigo foi apresentar uma abordagem atual, baseada em evidências, sobre a aplicação e indicação de corantes vitais durante a cirurgia vitreorretiniana. O emprego desses corantes possibilitou uma maior previsibilidade para a cirurgia, melhorando assim seu prognóstico pós-operatório. Apesar do campo da cromovitrectomia está em plena expansão de pesquisas, um corante gold standard para cromovitrectomia ainda não está estabelecido.

ABSTRACT

To report subretinal migration of indocyanine green dye (ICG) and subsequent retinal pigment epithelial (RPE) atrophy during macular surgery for serous macular detachment. A 65-year-old woman presented with residual epiretinal membrane and serous detachment of the macula following vitreoretinal surgery for epiretinal membrane. She underwent resurgery with ICG-assisted internal limiting membrane peeling and intraocular tamponade. Intraoperatively a large area of subretinal ICG was seen with subsequent RPE mottling and atrophy of the macula in the area involved during follow-up. This case demonstrates that subretinal migration of ICG is possible and can be toxic to RPE.

ABSTRACT

Objetivos: Descrever os aspectos clínicos, oftalmológicos e da videoangiografia digital com fluoresceína sódica e indocianina verde (VDIV) na coriorretinopatia esclopetária. Pacientes e métodos: Nove pacientes com o diagnóstico de coriorretinopatia esclopetária foram submetidos à avaliaçäo oftalmológica e, em 6 casos, complementada com a realizaçäo da videoangiografia digital com fluoresceína sódica e indocianina verde (VDIV). Resultados: O quadro oftalmoscópico típico encontrado foi de densas cicatrizes coriorretinianas, hemorragia vítrea e proliferaçäo fibroglial. Em um paciente também foi encontrada avulsäo do nervo óptico. Os achados da angiofluoresceinografia e da DVIV säo semelhantes, com extensas áreas de hipofluorescência por näo perfusäo, por bloqueio pigmentário e pelo tecido fibroglial nas áreas de cicatrizes coriorretinianas. Nos casos de traumatismo recente, onde há quebra de barreiras vasculares, pode ser observada hiperfluorescência por vazamento do contraste e hipofluorescência por bloqueio hemorrágico em ambos os métodos. Conclusöes: A coriorretinopatia esclopetária apresenta espectro variável dependendo do tipo de agente (arma, calibre, projétil, trajeto) e da sua interaçäo com os diversos tecidos oculares com diferentes resistências ao trauma. Embora o diagnóstico da coriorretinopaita esclopetária seja clinico-oftalmológico, este estudo descreve as alteraçöes da VDIV em uma série de casos complementando a avaliaçäo do paciente no trauma ocular.

ABSTRACT

Determinar el valor de la Video-angiografía con indocianina verde (V-ICG) en la evaluación de tumores de coroides. Se realizó en forma prospectiva la V-ICG en 151 pacientes consecutivos (ojos) con tumores coroideos, incluyendo 110 con melanoma maligno de coroides (MM), 25 con hemangioma circunscrito coroideo (HCC) y 16 con metástasis coroidea (MC). En el grupo de ojos con MM se observó una fluorescencia máxima a los 21.1 (rango: 0.4 a 66) minutos después de la inyección del contraste. Durante el momento de máxima fluorescencia el patrón varió de hipofluorescente (58 casos) a isofluorescente (32 casos) e hiperfluorescente (20 casos). El melanoma amelanótico de coroides mostró un inicio de la fluorescencia mas temprano que el MM pigmentado (p=0.04). En el grupo de ojos con HCC la hiperfluorescencia mas temprana se observó 0.5 (rango: 0.2 a 1) minutos después de la inyección del contraste, mientras que la máxima hiperfluorescencia se observó 3.7 (rango 0.5 a 11.7) minutos después de la inyección. En la fase tardía del estudio 18 (72 por ciento) casos mostraron un "lavado del contraste". En el grupo de las MC 12 (75 por ciento) de los casos demostraron una fluorescencia difusa homogénea con isofluorescencia tardía, mientras que 4 (25 por ciento) casos demostraron una hipofluorescencia temprana con persistencia de una hipofluorescencia menos intensa en la fase tardía. La V-ICC puede ser un examen complementario no invasivo útil en el diagnóstico diferencial de tumores coroideos
